PROFIL RESISTENSI INSULIN PADA PASIEN DIABETES MELITUS TIPE 2 BERDASARKAN LAMA TERDIAGNOSA

Abstract

Type 2 diabetes mellitus is a chronic hyperglycemia condition characterized by decreased insulin secretion and insulin resistance. As the disease progresses, physiological changes occur that can worsen insulin resistance and glycemic control. Therefore, it is important to understand the relationship between the duration of diagnosed diabetes and insulin resistance levels as the basis for more effective therapy management. This study aims to determine the relationship between the length of diagnosis of type 2 DM and the level of insulin resistance measured using the HOMA-IR index. The research design was observational analytics with a cross-sectional approach. Data included durations of DM (years) and fasting blood glucose levels and fasting insulin used to calculate HOMA-IR. The normality test was performed with Shapiro–Wilk, and the relationship between variables was analyzed using the Spearman correlation test. The results of the Spearman test showed a moderate and significant positive correlation between the duration of type 2 DM and insulin resistance with a correlation coefficient value (r) = 0.340 and p = 0.032 (p<0.05). This finding showed that the longer the patient suffers from type 2 DM, the higher the level of insulin resistance. Long-suffering DM plays an important role in the increase in insulin resistance, which reflects the progression of the disease.

 

Abstrak: Diabetes melitus tipe 2 merupakan kondisi hiperglikemia kronis yang ditandai dengan penurunan sekresi insulin dan resistensi insulin. Seiring perjalanan penyakit, terjadi perubahan fisiologis yang dapat memperburuk resistensi insulin dan kontrol glikemik. Oleh karena itu, penting untuk memahami hubungan antara lama terdiagnosa DM tipe 2 dengan tingkat resistensi insulin sebagai dasar pengelolaan terapi yang lebih efektif. Penelitian ini bertujuan untuk mengetahui hubungan antara lama terdiagnosa DM tipe 2 dengan tingkat resistensi insulin yang diukur menggunakan indeks HOMA-IR. Desain penelitian adalah analitik observasional dengan pendekatan cross-sectional. Data meliputi lama terdiagnosa DM (tahun) dan kadar glukosa darah puasa serta insulin puasa yang digunakan untuk menghitung HOMA-IR. Uji normalitas dilakukan dengan Shapiro–Wilk, dan hubungan antarvariabel dianalisis menggunakan uji korelasi Spearman. Hasil uji Spearman menunjukkan adanya korelasi positif sedang dan signifikan antara lama terdiagnosa DM tipe 2 dan resistensi insulin dengan nilai koefisien korelasi (r) = 0,340 dan p = 0,032 (p<0,05). Hal ini menunjukkan semakin lama pasien menderita DM tipe 2, semakin tinggi tingkat resistensi insulinnya. Lama menderita DM berperan penting terhadap peningkatan resistensi insulin, yang mencerminkan progresivitas penyakit.
